一期植骨联合肱骨近端锁定钢板治疗老年肱骨近端粉碎性骨折_医学论文 作者：戴培光 马爱兵 金莲芳 【摘要】 目的 探讨一期植骨联合肱骨近端锁定钢板(LPHP)治疗老年肱骨近端 粉碎性骨折的临床价值。方法 应用一期植骨、LPHP治疗老年肱骨近端粉碎性 骨折16例，对其治疗效果进行临床分析。结果 16例骨折全部愈合，无肱骨头 坏死病例;按照Neer百分制评定标准评定，结果优9例，良6例，可1例,优良 率为93.75%。结论 一期植骨可以很好的解决粉碎性骨折的骨缺损，增加内固 定的强度，促进骨折愈合;LPHP具有软组织剥离少，固定牢固可靠，术后可早 期功能锻炼，骨折愈合快，关节功能恢复好等优点。一期植骨联合LPHP是治疗 老年肱骨近端粉碎性骨折的理想方法。 【关键词】 植骨;锁定钢板;肱骨近端骨折 【Abstract】 Objective To study the clinical value of LPHP and bone graft treatment for the elderly comminuted proximal humeral.Methods Abone graft, LPHP the treatment of the elderly 16 cases of proximal humerus comminuted fracture and of its therapeutic effect for clinical analysis.Results All 16 cases of fracture healing, no cases of humeral head necrosis. Neer system in accordance with the assessment criteria percentile results: excellent 9cases, good in 6cases, example 1. Excellent rate was 93.75%.Conclusion A good solution bone comminuted fracture of the bone defect, increasing the strength of fixation to promote fracture healing. LPHP with less soft tissue dissection, fixed firmly reliable, can be an early postoperative functional exercise, fast fracture healing,good joint function recovery.LPHP combined with bone graft is an ideal way in the treatment of senile proximal humerus comminuted fracture. 【Key words】 bone graft ,locking plate,proximal humerus fracture 肱骨近端骨折是一种较常见的骨折，在老年上肢骨折中占第2位，是老年 人常见的创伤性疾病，有文献[1] 报道 75%的肱骨近端骨折发生在 60岁以上 的老年人，且女性发病率明显高于男性。对于无明显移位的肱骨近端骨折，保 守治疗能取得较好的疗效。然而对于移位明显的肱骨近端粉碎性骨折，特别是 伴有老年骨质疏松患者，仍然是临床骨科治疗的难题。2004年6月—2008年6 月，笔者应用一期植骨、肱骨近端锁定钢板(locking proximal humerus plate，LPHP)治疗老年肱骨近端粉碎性骨折16例，疗效满意，现报告如下。 1资料与方法 1.1 一般资料 本组 16例患者，男 6例，女 10例，年龄 60～78岁，平 均65.7岁。损伤原因：车祸伤8例，摔伤 6例，高处坠落伤4例;全部为闭合 新鲜骨折。骨折类型(按 Neer分型[2] )：Ⅲ型 11例，Ⅳ型 5例，合并肩关 节脱位2例。 1.2 手术方法 臂丛麻醉后，患肩垫高，取肩关节前内侧切口，分离胸大 肌、三角肌间隙，保护头静脉并连同部分三角肌牵向内侧 ，尽量保护关节囊、 肩袖组织，显露肱骨近端，明确各骨块的位置，根据骨缺损量和骨质疏松的程 度，取髂骨块，牵引肱骨将骨块插入远端髓腔，骨折复位，髂骨块的上端插入 肱骨近端并支撑肱骨头，复位大小结节及游离骨块，用克氏针临时固定，此时
