NOhasnumerousfunctionsinvasodilation,inhibitionofplatelet aggregation,anti-inflammation,anti-apoptosisandanti-proliferation, whichalltogetherplayanimportantroleinmodulatingthenormal functionoftheheart.TheseeffectsofNOarepredominantly mediatedbycGMP.However,inpatientswithheartfailure,platelets andcoronary/peripheralarteriesarehyporesponsivetothe antiaggregatoryandvasodilatoreffectsofNOdonors.NOresistance resultslargelyfromthereductionofNObioavailability.NO resistanceconstitutesanimpairedphysiologicalresponseto endogenousNOandmaycontributetotheincreasedriskof ischemicevents.ImpairmentinresponsivenesstoNOinpatients withheartfailureimpliesapotentialproblemthatthosepatients,in greatestneedofnitratetherapy,maybeleastlikelytorespond.
